I try to explain what I think not right with my summicron collapsible.

When I fit it on my IIIF, the small black triangle (which mark the center position) on the depth of field scale is not at 12 o'clock position of the camera. Now it's staying at a position between 1 and 2 o'clock.

Though I know it's quite common for voigtlander lens, I don't expect it with a leica lens.

Anyone has similar issue?

The index marks don't normally line up at 12.00 as a practical consideration. When an accessory is put on the shoe, the view might be blocked and the marks won't be seen. Usually, the index mark is slightly askew to the right (when seen from the above). This is usual for lenses with engravings on short barrels like the collapsibles.

Totally normal, and it will line up a little differently on each camera.
