1.) Is there any 3rd party that can do the 6-bit coding on Leica lenses?

2.) Does coding affect focus? (I can't recall where, but I read that it does)

3.) Any info on something called the "pen method" of coding?

It's patented, so no-one can do it fully and legally, though there are some fudges around.

1. See above. And if you supply a template, any competent machine shop can do it for you. I know one dental technician who coded all his lenses himself. (i.e. BenZ)
2. No it does not. However, as Leica exchanges the mount for a coded one, it may be that in exceptional cases the tolerances may affect the focus. In 99.9 % of the cases there is no difference, except for the better. Leica checks and repairs the lenses as they come in.
3. You make the appropriate black marks on the lens mount yourself and the camera is fooled into thinking it is a coded lens.
I'll add a 4.
If you are not hung up on getting the focal length in your EXIF data, coding is not needed for lenses of 50 or longer, as there is no need for the camera to apply corrections to the image.

There is one error on the Popflash page. The manufacturer is Match Technical (Tim Isaac) not "Generic". http://www.matchtechnical.com

Glad to see PopFlash carries the Match Technical coder, as it seems like a good though temporary solution.
